Most people hurt in a car accident start with 2 to 3 chiropractic visits a week, then taper to 1 to 2 a week as symptoms improve. For a moderate to severe injury, plan on about 8 to 12 weeks of active care; complex cases can take up to 6 months. Your exam and re-exams set the schedule.

The short answer

Injury Typical start Typical length
Mild injury (Croft grade I) 2 to 3 visits a week, tapering Croft maximum about 11 weeks, 21 visits; many need fewer
Moderate to severe injury 2 to 3 visits a week, tapering to 1 to 2 About 8 to 12 weeks of active care
Complex soft-tissue or post-concussion Set by your exam Up to 6 months, with re-exams

Your plan is set after your exam and adjusted as you heal. It is not a fixed package.

Why visits start frequent and then taper

In the first 72 hours (Phase 01 Inflammation), swelling builds and muscles guard the injury. Through Phase 02 Repair, 2 to 3 visits a week calm pain and restore motion while the tissue heals. As you improve, visits space out and shift toward strengthening exercises you also do at home. The goal is to need us less every week.

A typical timeline

  • Phase 01 Inflammation (0 to 72 hours): we calm inflammation and protect the tissue with electrical stimulation, soft-tissue work and gentle adjustments.
  • Phase 02 Repair (72 hours to 12 weeks): most active soft-tissue healing happens here; adjustments continue, stabilization exercises begin, and visits taper as your findings improve.
  • Phase 03 Remodel (12 weeks to 12 months): tissue keeps maturing under load after pain eases; progressive rehab and return to activity, with visits tapering toward independence.
  • Phase 04 Living Your Highest Lifestyle: as-needed visits and periodic re-checks.

The phases are the framework, not the schedule; your recovery may be faster or slower.

How often for whiplash

Usually 2 to 3 times a week at first, tapering to 1 or 2 a week as your symptoms improve. Your exam and injury grade set the plan; re-exams decide when to taper. Our doctors follow the Croft treatment guidelines, which set the upper limits for each grade of whiplash; many patients need fewer visits. Someone with mild whiplash after a low-speed crash needs fewer visits than someone hurt in a major collision. Treatment usually combines gentle adjustments, soft-tissue work and neck exercises. What changes how long you need care

  • How severe the injury is, and whether discs or nerves are involved
  • How soon you start: starting soon after the crash, once serious injury is ruled out, lets us document your injuries and begin your plan early
  • Your age, fitness and any earlier neck or back problems
  • How consistently you keep appointments and do your home exercises

How you know you are done

Your chiropractor re-examines you along the way and re-scores the same symptom questionnaires from your first visit. When your scores and exam show you have reached your goals, or have stopped improving, you are released or referred on.

Do not stop the moment you feel better. Feeling better is a milestone, not the finish line. Injured tissue keeps remodeling for up to 12 months after pain eases, which is why your visits taper instead of stopping all at once.

Who pays for the visits

After an Oregon crash, your own auto policy’s PIP coverage pays reasonable and necessary care, at least $15,000 per person for care within two years (ORS 742.524). We verify your coverage and bill PIP directly. More: Oregon PIP coverage.
