How we document
- Initial exam: mechanism of injury, history, objective findings and validated outcome measures (Neck Disability Index, Oswestry, Bournemouth neck and back).
- A written treatment plan with measurable goals and an expected course of care.
- Re-exams that re-score the same measures, so progress or plateau is on the record.
- A discharge exam with final measures and any future-care recommendations.
- Narrative reports on request, written for adjusters, IME examiners and counsel.
Oregon PIP, handled
- We verify PIP before the first visit and bill the PIP carrier directly.
- PIP pays reasonable and necessary medical expenses incurred within two years of the injury, at least $15,000 per person (ORS 742.524(1)(a)).
- Charges are presumed reasonable and necessary unless the insurer denies them within 60 days of the provider’s claim (ORS 742.524(1)(a)). A denial must be in writing, with the reason and how to contest it (ORS 742.528).
- We answer insurer questions in writing within 10 business days, as ORS 742.524(1)(a) requires.

When PIP runs out or is denied
- Care continues on a signed lien or letter of protection with your agreement.
- Where it applies, we coordinate the patient’s health insurance.

- Records and billing ledgers on request, with the client’s signed authorization.
